Managed care/HMOs usually require a referral, which needs to be arranged prior to you initial visit. TMJ appliances may be covered under your medical plan and usually require pre-authorization. Patients who will be undergoing extensive procedures in the office or any hospital surgery usually require a pre-determination of benefits after an initial consultation.
